Repair authorization & direction to pay
1. Authorization to repair
I hereby authorize the above repair facility to perform all work necessary to restore my vehicle to its pre-loss condition as described in the written estimate. I further authorize the facility to perform any supplemental repairs discovered during teardown, provided I am contacted and give oral or written approval before supplemental work proceeds.
2. Direction to pay
I hereby authorize and direct my insurance company, and any other insurer or responsible party obligated for this claim, to issue payment for all covered repairs directly to the above repair facility. This authorization remains in effect until all repair obligations are satisfied.
3. Misdirected payment
In the event any insurance check, draft, or electronic payment is inadvertently sent to me rather than the repair facility, I agree to (a) notify the facility immediately and (b) deliver said payment to the facility within 24 hours of receipt, unendorsed and undeposited.
4. Deductible and outstanding balance
I understand I am solely responsible for my applicable deductible and any amounts not covered by my policy, including depreciation, betterment, or non-covered operations. All balances are due upon repair completion and prior to vehicle release.
5. Personal property and test drive
I understand the facility is not responsible for loss of or damage to personal property left in or on the vehicle. I authorize the facility to operate my vehicle on public or private roadways as needed for inspection and quality verification of repairs.
